Given what you said about the fan, I don't suspect software at all.
I'll bet that the fan is dying, and that your computer is overheating.
My first step would be to replace the fan (or possibly replace the
power supply that contains the fan).

You might try running with the cover off until you've replaced
the fan and seeing if that makes a difference. If it does, then
you're almost certainly overheating with the cover on.
