It depends on the firewalls inbetween the two computers.
Normally a router will not block 3389 (the Remote Desktop
Port) outgoing. However the router at your home, for example,
will have to forward port 3389 incoming to the computer you
want to connect to. Exactly how you do that will vary based
on what router you have.

It's also possible that your company has 3389 outgoing
blocked, you may want to check with them.

But in general, Remote Desktop can work across routers
just fine. I do it myself across two routers quite often.
